Eligibility for Services
Outreach serves Polk County, North Carolina individuals and families experiencing crisis.
Financial assistance is provided through direct payments from Outreach to vendors. Funds are never distributed directly to individuals.
Food assistance is provided through our on-site client-choice food pantry, where clients are able to shop for items that meet their household needs.

To Qualify for Services, Clients Will:
- Meet with an Outreach representative to complete an intake interview
- Provide verification of Polk County residency (such as a current bill showing a home address)
- Present a valid photo ID
Additional requirements for financial assistance:
- Proof of total household income for all household members
- The past-due bill for which assistance is being requested
