Financial Assistance
Outreach serves Polk County, North Carolina individuals and families in need or experiencing crisis. Financial assistance is provided through direct payments from Outreach to vendors. Funds are never distributed directly to individuals.
Qualifications for Financial Services
To qualify for assistance, you must:

Meet with an Outreach representative to complete an interview

Provide proof of Polk County residency

Current bill showing home address

Drivers license
-
ID Card
-

Present valid photo ID
-
Proof of total household income for all household members (i.e. paycheck stubs, social security statements, disability statements)

All bank statements

The past due bill for which assistance is being requested
This assistance is provided to individuals and families to help meet critical, basic needs. Our financial assistance support includes rent, mortgage, utilities and home repair.
